Here's how to add a custom “Profile Field” to an assessment, starting from the "Account" tab of your Dashboard.

 

Step-by-Step Guide:

Step 1: Click the "Assessments" tab in the left side menu of your "Account" page.

 

Step 3: Make sure you have at least one custom profile field created.

Step 4: When you are creating a new assessment or editing an old one, you can then select the custom profile field via the auto-complete search box when clicking "Add a Field."

 

Step 5: Save Assessment.
